For many of us, completing paperwork has become a required, and sometimes "painful," part of doing business. Prior to your surgery, you will be asked to read and sign a consent form. The consent form will help to inform you of many of the possible risks involved with the surgery. You must be at least 18 years old in order to sign the consent form. If you are not 18 years of age, a parent or guardian must sign the form for you. Please keep in mind that if you will be taking any types of pre-medication prior to surgery (other than antibiotics), you will need to sign the consent form prior to taking this medication.
Below is a sample of the consent form you will be asked to sign. Please take the time to read it. Although the information asked is not meant to alarm you, we realize that it sometimes can do just that. However, please remember that the incidence of any negative, long-term effects occurring during or after your surgery, is very, very low.
